As an FSAStore.com customer (“Customer”, “you” or “your”) you have access to the following services:

• Order your over-the-counter prescription through our prescription fulfillment partner, Wellpartner.
• Order over-the-counter products and other non-prescription products.
• Search FSA-eligible specialties and learning center.
• Track the status of your order(s).
• Submit questions to a Wellpartner pharmacist.

We understand the sensitive nature of the medical, financial and personal information that you may share with us. We are committed to protecting your privacy and ensuring that any of your personal information shared with us remains confidential.

Information We Collect and Use

Upon receipt of a valid order, we will require basic information, such as your name, billing address, mailing address, phone number, and credit card and/or other payment information to process your order. This information allows us to correctly process and fill your order, as well as notify you of your order status. With respect to over-the-counter prescription orders, our pharmacy fulfillment partner Wellpartner is required by law to obtain sufficient information to properly screen and dispense over-the-counter prescriptions. This information may include, but is not limited to, the name of your prescribing physician, your gender and any drug allergies and any existing medical conditions you may have.

We also obtain financial information that you may enter into our online services (including account numbers, logins, passwords and transactional information). Your personally identifiable financial information is confidential and is solely used to provide and maintain the services you have requested. To protect access to your account we may also ask you to verify your identity in the event you contact customer support for assistance with your account. This information will only be used to verify critical information about you, including your contact information in relation to your account. Currently, there is no need for you to submit health insurance information. We do not accept insurance or other third-party payment for over-the-counter prescriptions.

Information Collection and Use by Third Parties

We use the personally identifiable information you provide to us only to fill orders and to send you information about our products and services.

In some cases, we use third party service providers to provide a product you order, to fulfill a service you request, or to market one of FSA’s products or services. For example, we may use third-party service providers to provide customer support or to host our websites. Third-party service providers who provide services and collect information on our behalf must comply with the practices outlined in this privacy policy.

We may compile and share aggregate statistical information regarding our Customers, sales, website traffic patterns, and website usage with select third-parties. In addition, we may use aggregate statistical information in conjunction with marketing and analysis tools so that we can better understand our Customers' needs and preferences and tailor our products, services and content accordingly. This marketing and analysis information is for our use only and will never include an individual customer’s medical, financial or personal information.

Except as described above, we will not release any personally identifiable information to any person or party without your permission and as otherwise permitted by law. This includes prescription information, financial information, health coverage information and/or personal information. We do not sell our customers’ personally identifiable information to third parties.

Security

The security of the medical, financial, and personal information you provide is of the utmost importance. We have implemented both technical safeguards for our systems and physical safeguards at our facility to ensure your information is protected from unauthorized use and disclosure. We have also developed a code of conduct for our employees who are authorized to access our Customers' personal information to ensure appropriate and consistent safeguards are followed for all our Customers. We use commercially accepted security practices to protect your personal medical and financial information. We regularly evaluate such security practices and update the same as appropriate. We do not store long-term financial information unless requested by you, and we have installed and applied our encryption technology and installed an SSL certificate to minimize unauthorized access to your information (see “Website Security” below).

Notification of Changes

We will use information in accordance with the privacy policy under which the information was collected. The respective websites www.wellpartner.com and www.fsastore.com may be updated from time to time to reflect changes to this privacy policy. We encourage all Customers to visit our sites frequently and to make note of any such policy changes.

Details on Our Privacy Policy — Website Information

You can visit FSAStore.com and access the following basic functions without revealing any medical, financial or personal information: browsing FSA eligible and FSA eligible-with-prescription products, visiting our FSA services section, and referencing our FSAStore.com learning center.

To access and utilize other FSAStore.com resources, such as the over-the-counter prescription processing functionality, you are required to register, open an account and provide all information which is required by FSAStore.com and our fulfillment partner, Wellpartner to process an over-the-counter prescription order. You will choose a user ID and a password so that we can verify your identity and determine that you are entitled to access the services for the referenced account. Except as necessary to process your benefits or assist you with your over-the-counter prescription orders, information you supply to us as part of the registration process and your subsequent use of the website as a registered user remains confidential and will not be shared with any third-party, including your insurance provider.

Over-the-Counter Prescription History Data

FSAStore.com’s fulfillment partner, Wellpartner, will store and manage your prescription history information pursuant to the same privacy policy and secured by the same technology that we use for your medical, financial or personal information.

Customer Communication

We will contact you by email when you first register with us and will also send you alerts via e-mail about your over-the-counter prescription order status, updates to your account, or other information about products and/or services. We will never send you promotional e-mail communications without you being able to easily opt out. We will also answer via e-mail or pre-recorded telephone communication, any questions you may present, notify you if there is a problem with your order, or communicate any pertinent information regarding your account.

From time to time we may send out email messages alerting you to products that are available, to remind you about providing feedback or to tell you about promotional offers. In addition, you will receive communication from us (1) if you contact us for a particular reason, (2) to receive your orders, or (3) because we need to contact you for administrative purposes such as winning a contest. Administrative e-mails will not contain promotional materials. You will always be able to opt out of receiving messages from us.

Other Stored Information — Log Files

We may keep a log of IP addresses, browser types, and access times to analyze trends, administer the website, track movement, and gather broad demographic information. We utilize this information to conduct website performance evaluations, to see where website visitors are coming from and to keep track of click stream data (the screens our users visit on our website). This data helps us to determine what content our customers find most appealing so that we can maximize their enjoyment of the website. This information is only used in the aggregate for statistical analysis purposes, and the IP addresses are not linked to personally identifiable information.

This website contains links to other websites. Please note that when you click on one of these links, you may be leaving our website and entering another website. Our Use of Cookies

Cookies are pieces of information that your browser stores on your computer's hard drive to identify you when you visit a particular website. Wellpartner uses browser cookies to track order identification. We use session cookies to provide a seamless experience on the website and to combine with our log files so that we can understand our website traffic and analyze our demographic information in aggregate form as well as track orders placed. If you have set your browser to reject cookies, our website will not identify you when you enter and you will need to manually type in your user ID and password each time you visit our website.

Website Security

We use a secure internet web portal / pages to take all orders and to display Customer information. All Customer information is stored in a firewall secured database. Customer information is available through individual accounts, which is only accessible by entering a customer specific user ID and password. All data is transmitted to Wellpartner through secure file transfer protocols.

In addition, we use Secure Socket Layer (SSL) software, a security standard supported by common web browser software, including Microsoft Internet Explorer (6.0 and higher). Sensitive information such as a Customer’s health data, address, and financial payment information is encrypted using up to 256–bit encryption for transmission over the Internet. When Customers place orders online or use the over-the-counter prescription process, all personal information and financial payment information related to such transaction shall be encrypted using secure file transfer technology when sent over the Internet.

Customers’ sensitive information, such as your password, allergy and medical conditions, and prescription history is stored and encrypted. While select departments at our pharmacy partner, Wellpartner will have access to basic contact information and aggregate demographic information, only those departments that need to access more sensitive information can view Customer health and insurance information. We also have additional security measures in place to protect the loss, misuse and alteration of the information under our control.

Correcting and Updating Personal Information

If you would like to review the personal information you have provided to us on this website, please log onto your account at FSAStore.com. To correct or update your personal account information select the option on the screen to “Edit Your Profile”, which appears along the right side of the page. Once you have made all corrections and or updates, remember to click the “Save” button to save them.
